Part-Time position focus is on supporting the integrated and collaborative primary care team – medical provider, behavioral health consultant, case manager, and nutritionist – to provide high quality care to patients with behavioral health needs. This position is a consultative role and assist the team by ordering, interpreting, and following up on diagnostic assessments. Document all care in the electronic health record in accordance with accepted medical/legal standards. Provide all medical care in accordance with accepted practice standards, hospital/clinic protocols (including supervision by a physician), and State licensing guidelines.
Provide assessment and medication consultation for primary care and behavioral health providers at all of Spectrum’s health centers when mental health, developmental, or substance abuse disorders are present and beyond the scope of the primary care provider’s practice.
Support the stabilization of patients discharged from mental health hospitalization back into primary care for follow-up and ongoing management.
Advise the prescribing medical provider on appropriate medications and other treatment strategies. Provide educational support on diagnosing.
Work with behavioral health providers in prioritizing patients for case review and treatment adjustment.
Participate in case reviews or curbside consultations either in person or by phone, making medication recommendations within 24 hours of the review or consultation
Attend required clinical meetings and trainings.
All other duties as required by licensure and the agency that will assist in the care and management of members presenting with psychiatric issues or illnesses.
Experience and Skills:
PMHNP from an accredited program
Current licensure to practice professional nursing in the Commonwealth of Pennsylvania
