Spectrum Health Services, Philadelphia – Psychiatric Nurse Practitioner

The part-time PMHNP at Spectrum Health Services (SHS) will address the mental health needs of behavioral health clients including psychiatric evaluation of clients, prescribing and managing psychotropic medications, as well as care management within the Patient Centered Medical Home (PCMH) integrated model with close coordination with other behavioral health and primary medical providers; assessing and assisting with staff training needs. The incumbent will be certified (waivered) in the treatment of drug addiction. The PMHNP reports directly to the CMO. Oversight for the PMHNP’s clinical practice will be provided by the CMO.

Duties and Responsibilities

• Expected to be proficient in the use of psychopharmacology
• Uses current evidence based practice, research and clinical knowledge in their role as PMHNP to improve patient outcomes and reduce complications
• Behavioral Health documentation must meet all regulatory requirements made by licensing and third party agencies and must be accordance with SHS Behavioral Health Policy and Procedures
• Coordinates with other medical specialties in the detection and treatment of psychiatric patients’
• Conducts mental health histories and assessments
• Formulates mental health treatment plan and manages acute and chronic mental health needs within PCMH model of care
• Provide treatment to clients with drug/opioid addiction
• Initiates appropriate referral for services based on clinical assessment
• Provide assessment and medication consultation for primary care and behavioral health providers at SHS for patients with complex mental health, developmental, or substance abuse disorders
• Examines patients to determine general physical condition, following standard medical practice
• Organizes data concerning patient’s family, medical history, and onset of symptoms obtained from clients, relatives, and other sources
• Orders laboratory and other special diagnostic tests and evaluates data obtained
• Administers psychotropic medications and therapeutic agents, necessary to implement a treatment, disease prevention, or rehabilitation regimen ordered by and within the scope of licensure
• Participates in periodic clinic staff conferences to present case histories; confer with internal external members of therapy team to develop comprehensive evaluations or plan pertinent therapy programs to provide maximum benefits for each patient
• Must participate SHS clinical meetings and staff training
• Support the stabilization of patients discharged from mental health hospitalization back into primary care for follow-up and ongoing management
• Consult with SHS medical providers on alternative or appropriate therapeutic drug regimens and other treatment strategies
• Identify learning needs of various populations, including providers and contribute to the development of educational programs to meet those needs
• Serves as patient/family advocate and liaison for clear communication of needs and concerns
• Work with behavioral health providers in prioritizing patients for case review and treatment adjustment
• Participate in case reviews or curbside consultations either in person or by phone, making medication recommendations of the review or consultation
• Attend required clinical meeting and trainings.
• All other duties as required by licensure and the agency that will assist in the care and management of members presenting with psychiatric issues or illnesses
• Completes other duties as may be assigned by the immediate supervisor that are consistent with the responsibilities of the position.
Minimum Requirements
• Graduation from accredited master’s level Nurse Practitioner program with a specialty in psychiatry
• Licensed to practice as nurse practitioner in Pennsylvania.
• BLS certification required
• PMHNP from an accredited program
• Work Experience in Behavioral/Mental Health
Other Skills & Knowledge

Incumbent required to be self-motivated and be able to work independently. Requires travel to three SHS sites and the physical demands associated with clinical nursing
