The RN Care Coordinator Team Lead is a Registered Nurse working in our care coordinator department, responsible for the daily supervision of care coordination functions and supervision of staff. The RN Care Coordinator assures the efficient delivery of coordination services to all clients in an ambulatory, primary-care clinic setting. Acts as a liaison in the coordination of care between the health center and the patients and helps to facilitate and connect patients to health and social services needed to improve health outcomes. Coordinates with Specialty Practices to ensure effective patient referral process and care coordination relationships.
Must be a graduate of an accredited nursing school, successfully completing state board and must have license to practice as a Registered Nurse in the Commonwealth of Pennsylvania. At least two years’ experience working with children and families in Public Health, Outreach, Social Work or related field. Prior experience with care coordination/care management in a medical setting is desirable, but not required if other relevant experience can be demonstrated.
